Medicare Part D is administered by private insurance companies. These plans can have different formularies (drug lists) so depending on which plan you are considering Xarelto may or may not be covered.
The good news is that this is pretty easy to find out by putting a list of your medications into the planfinder tool at medicare.gov. This tool will look at the different plans, their formularies, pharmacy network, and the cost sharing for your particular medications and make a list of the plans available sorted by "total annual cost"
