The top-tier plan is the Platinum Plan. With this plan, clients have no overall annual limit for select covered services, no overall deductibles, and services such as routine foot care, acupuncture, bariatric surgery, and routine hearing tests are covered.
The next plan is the Gold Plan. With no annual maximum limit on select covered services, this plan covers routine hearing tests, chiropractic care, routine eye exam for adults, and routine foot care.
With the Gold Plan, there are overall deductibles of $1,000 per person, which do not apply to preventive care services. There is an out-of-pocket limit of $6,350 per individual for in-network providers.
The Silver Plan is next, with overall deductibles of $2,500 per person. This does not apply to preventive care services and prescription drugs. The deductibles for prescription drugs are $250.
There is no annual limit for select covered services. These services include routine hearing tests, private-duty nursing, and hearing aids. Out-of-pocket limits are up to $6,250 per individual for in-network providers.
The Bronze Plan is the most basic plan. There are overall deductibles of $5,000 per individual, which do not apply to preventive care services.
Members of this plan have an out-of-pocket limit of $6,350 for in-network providers. Services covered include bariatric surgery, acupuncture, routine foot care, and routine hearing tests. There is no overall annual limit on select covered services.
